Since it tends to be the most time- and emotion-consuming part of the process, we suggest your first step be to contact the Registrar’s Office of the institution from which you received your highest degree to date.

Either they or you need to then send at least a hard-copy – scanned copies are not acceptable – of your complete transcript, which must include all the courses you took / your grades for those courses / and clearly indicate the date on which you received your degree.

Some institutions have related forms – e.g., Certificate of Attendance – that you may also feel free to send along as well to the following address:
